It should not take more than 2-3 minutes to complete the form. I will respond to you by email as soon as possible, usually within 24 hours. At that time, I will let you know if we may be a good fit and how you may schedule an appointment. I will also answer any questions you may have.
I am accepting self-pay patients and beneficiaries of select Insurance plans. I am a network provider for Medicare, Aetna, United Healthcare, United Behavioral Health, and Optum.
I am also an Employee Assistance Provider for select EAP programs.
